We offer two simple ways to receive primary are. Choose the option that fits your needs and how you prefer to pay:
OPTION 1: Primary Care using Insurance
Currently we accept: Atena, CIGNA & Medicare B
Pending plans: BCBS-IL, Humana & Health Link.

Ocean Breeze Wellness and Primary care, we offer personalized weight, loss plans designed to help you feel energized, confident and in control of your health. We will work together and create a tailored treatment plan to support lasting wellness - not quick fixes.
There are many medication options to choose from. Not all medication is created equal, and not all medication is going to have the same effect for everyone.
Monthly Membership $150
Monthly cost of medications varies: $100 to $325/month addition to the monthly membership. Price range varies based on medication, dose, shipping etc. Each medication is ordered on an individual basis after payment has been received.
Medications we may prescribe based on eligibility:
GLP-1 medication’s: Semaglutide & Tirzepatide
Other options: Phentermine, topiramate, Contrave
All weight loss medications require an evaluation and may not be appropriate for everyone.

Please note that peptide therapies are considered investigational and are not approved by the FDA for all uses. However, many peptides have shown promise during research and clinical experience. They are compounded and prescribed off label based on provider discretion and patient needs. Peptides are not intended to diagnose, treat, cure, or prevent any disease.

Available services include:
Sick visits/minor acute (non-emergent) care $100
Wellness Exams and physicals $125
Basic vitamin injections such as B12: one for $35 or 5 for $150 - a savings of $25
Lab’s: pricing based on tests ordered
Telehealth visits $75
Home visits $25 additional cost, unless on membership.
Medical Cannabis Certification (Illinois) $150 initial visit $100 Renewals

At Ocean breeze wellness in primary care, we focus on proactive, personalized care- but there are certain services that we do not offer.
Currently, we do not take insurance, but we are working on this process - if you have a specific Insurance you’d like us to look into, please drop us a request.

Frequently Asked Questions
Do you accept insurance?
At this time, we are working with insurances to be “in-network”. However, we are a direct primary care practice first meaning not accepting insurance allows us to keep care simple and affordable. Many patients use their insurance for medications, imaging, specialist care, and labs outside of what’s included. HSA/F SA funds can often be used for membership. For those “out of - network” we can provide a super-bill for you to submit to your insurance for possible reimbursement.

What if I Need care after hours
we are secure messaging until 9 PM weekdays and 9 AM to five on weekends. And on-call fee may apply. For emergencies always call 911

Hi! I’m Amanda Hildebrand, founder of Ocean Breeze Wellness & Primary Care.
I’ve been in healthcare for over 14 years working in various roles and departments across the hospital and clinic setting.
Those experiences gave me a broad perspective and a deep appreciation for the many ways people’s health journeys can look.
For the last five years as a Nurse Practitioner, I have focused on building strong lasting relationships with patients - Listening closely, tailoring care to the individual and making sure no one feels like just another chart.
I started Ocean Breeze Wellness and Primary Care because I wanted to bring together the best of what I’ve learned: The clinical knowledge, the personal connection and the belief that healthcare should feel approachable, compassionate, and centered around you.
When I’m not caring for patience, I usually spend my time with my family. I love watching my boys play baseball and finding inspiration in the little things that make life feel balanced and whole.
My mission: To create a welcoming space where your health and wellness can truly thrive.
